Booking Your IV Therapy Session:
New patients must complete the Health History and Consent Form prior to their first appointment so our clinical team can review and clear you for treatment, helping prevent any delays at check-in. Forms must be submitted at least 24 hours prior to your appointment. Please be specific when listing all current medications, supplements, and injections (including dosages if known), as incomplete information may delay treatment clearance.
Completed forms may be returned by email at the patient’s discretion, understanding that standard email communication is not HIPAA-secure. Patients who prefer not to send forms electronically must drop off completed paperwork in the office at least 24 hours in advance of their appointment.
A $150 deposit is required for IV infusions and a $95 deposit is required for NAD injections at the time of booking; regular IM injections do not require a deposit.

How to Book:
1) Select 'Book an Appointment'
2) Make sure you're selecting the Physical Synergy Location, otherwise it will not allow you to move forward. Choose your IV Therapy service.
3) Select the day and time that you'd like.
4) Book your appointment and you're all set!
